    Garden City Healthcare Center is proud to be a four-star, Medicare-certified, short-term rehabilitation and skilled nursing home located in Modesto, California. We are currently looking for a full time Ward Clerk to join our team.

    Ward/Unit Clerk


    Policy: The Ward/Unit Clerk position is designed to aide in the flow of information within the facility between resident’s, family members, nursing staff, medical records staff, physicians, and all other interested parties. A Ward/Unit Clerk should be assigned to each nursing station, work from the nursing station, and ideally this position should be filled seven days a week. The management of this position falls under the Medical Records Supervisor with respect to the assignment of duties and for the purposes of annual performance evaluations.


    Ward/Unit Clerk Duties


    The duties of the Ward/Unit Clerk can be diverse and varied, but should always be designed with the ultimate focus of aiding the facility in the flow of information. A sample list of appropriate duties are:


    1. Answer telephones at the stations
    2. Aide the physicians, nurse practitioners and consultants with access to resident information whether it be in electronic (Matrix) or paper (Thin Chart) form
    3. Fax to physicians, pharmacy, etc
    4. Conduct daily review of Thin Charts to ensure all charting is in the appropriate location (Matrix) and scanned into Matrix as needed
    5. Transcribe hospital transfer orders into Matrix for verification by admitting nurse
    6. Start consents for all psychotropic meds.
    7. Print resident specific flow sheets as needed
    8. Complete the following audits:
    9. Orders Verified Audit
    10. Daily PRN narcotic Audit
    11. Lab Due Audit
    12. Lab Completed Audit
    13. Weight Audit with progress note and notification to MD and RP
    14. Nursing Lab Compliance Audit (as assigned by the DON).
    15. Maintain a well-organized and clutter-free environment at the nurses station, including maintaining the organization and cleanliness of the Physician’s Documentation Room.
    16. Provide data-entry support to the nursing staff related to the documentation of vitals in Matrix as needed
    17. Set up Patient appointments
    18. Prepare documentation for resident appointments and referrals, secure transportation for all resident appointments and outings. Carry out any new orders from returned appointments
    19. Print documentation for emergency discharges
    20. Print documentation for planned discharges
    21. File appropriate documents as needed into Thin Chart
    22. Pull Thin Chart for discharged residents and deliver to Medical Records office
    23. Monitor fax status of all orders in Matrix and correct as needed
    24. Monitor resident alerts and notify charge nurse when applicable
    25. Assist the Nursing Supervisor/ADON with periodic revision of the CMS 802 report.
    26. Act as a liaison between Families and Medical Records Department when resident information is requested.
    27. Print care plan’s for weekly summaries that are due and give to nurses.
    28. Print daily bowel list and give to nurses (run report going back 3 days)
    29. Check crash cart weekly that all required items are in the cart for emergencies.
    30. Check fridges daily for any items that are not of that day’s date
    31. Medication Return Log with Proper Documentation completed correctly
    32. Maintain station cleanliness by keeping all drawers organized, countertops clean, phones and computers sanitized on a daily basis, and general up keep.
    33. Weekend duties: include all of the above and
    34. Print out alerts and put in assignment book
    35. Run report for previous days admissions and events and audit for complete assessments and corresponding care plans.
    36. Function as Medical Records “agent” at the nurses station, with attention to ensuring the quality and completion of documentation, and any subsequent documentation audits. This includes; compiling records to assist the Medical Records Supervisor with the appeals process, maintaining resident door labels, and maintaining the unit’s notification board
